1993 COMPUTER SCIENCES DEPARTMENT APPLICATION REQUIREMENTS The Computer Sciences Department considers applicants for graduate study once a year. Applicants are admitted for fall semester only. To be considered for admission in Fall 1993, we must receive your com- ---- plete application by January 15, 1993. To be considered for a Univer- ----- ----------- sity Fellowship, the deadline for all application materials is December 20, 1992. We will inform you of our decision by April 1. Certain items must be sent to the Graduate School and other items to the Department, as follows. Send items (1)-(3) to: The Graduate School, 228 Bascom Hall, Univer- sity of Wisconsin, Madison, Wisconsin, 53706. (1) APPLICATION FORM and PERSONAL DATA SHEET: The Graduate School application has two parts: the application itself, and a personal data sheet. The originals of both parts go to the Graduate School. (2) APPLICATION FEE: The Graduate School requires that all applicants pay a fee of $20. Payment of the fee must be made in US dollars. You may pay by check or money order, payable to the University of Wisconsin-Madison. Do not send cash or any type of coupon. Note that the application form and the fee must be sent in the same -- --- ---- envelope. -------- (3) OFFICIAL SCHOOL RECORDS: Ask your registrar to send an official transcript, including grade point average and a statement of class rank. If your university does not issue transcripts as such, you must send official documents, signed by an administra- tor of your university, indicating the following: (1) year of admission, (2) number of years enrolled at the university, (3) reference to quality of work and analysis of grading system, (4) evidence that exams were passed, (5) diploma showing certifica- tion of degree, class and year, (6) General Certificate of Educa- tion or an equivalent. (Official school records must be sent to ---- the Graduate School in order for your application to be pro- cessed.) (4) COPY OF APPLICATION FORM: The blue copy of the application form must be sent to the Department. (5) STATEMENT OF PURPOSE: A brief essay, no more than one page, about why you are interested in graduate study and what you propose to do in graduate school. (6) COPIES OF SCHOOL RECORDS: An copy of your official transcript (with grade point average and class rank) must be sent to the Department. (7) LETTERS OF RECOMMENDATION: We require three letters of recommen- dation whether you are requesting financial aid or not. (8) GRE SCORES: All applicants must have official Graduate Record Examination (GRE) scores sent to the Department. We require ver- bal, quantitative, and analytical scores, plus the advanced GRE in any field. To make arrangements, contact: Educational Testing Service, Box 955, Princeton, New Jersey 08540. Processing of exam scores takes approximately six weeks. On your application, please tell us the date of your exam and your scores, if known. (9) TOEFL/TSE SCORES: Applicants whose undergraduate education was not primarily in English are required to take both the Test of English as a Foreign Language (TOEFL) and the Test of Spoken English (TSE). Both are offered by the Educational Testing Ser- vice. Official scores must be sent to us by ETS. (10) OTHER MATERIALS: Applicants with prior professional experience (including teaching experience) should include a resume giving information on positions held. This information may be used, for example, to determine Teaching Assistant salary. To aid us in planning for our incoming students, please fill out and return our Interest Area Questionnaire. Special Student status allows you to attend classes, but not to receive a degree. Admission requirements for Special Student status, for the summer or other semesters, are less stringent than those for admission as a regular student. If you are applying for re-entry to our graduate program, or are requesting that we reactivate a previous application, please send the following to the Department: (1) up-to-date transcripts, (2) letters of recommendation no more than one year old (preferably new), (3) an up-to-date statement of purpose and (4) a new Interest Area Question- naire.
